Physicals: All students are required to have a complete PIAA Physical Form completed before any participation in practice or competition can occur. Section 5 & 6 of the PIAA physical forms can be picked up at either the CASHS, CAMS North, or CAMS South offices or by clicking HERE to download the most updated form. Only Section 6 needs to be completed by hand as sections 1-5 are now completed via FamilyID. PLEASE NOTE; however, that physicians will want to see a completed Section 5 before they will sign off on Section 6. Only Section 6 needs uploaded to FamilyID though.
The sports seasons begin: Fall (August 7, 2023 for football and August 14, 2023 for all others); Winter (November 18, 2023); and Spring (March 6, 2024). Any physical received after June 1, 2023 will be accepted for the entire school year. A physical received prior to June 1, 2023 will not be accepted for the 2023-2024 school year.
Re-certifications (Section 7 & 8): Any student participating in a Winter or Spring sport that previously participated in a sport during either the Fall or Winter seasons will need to complete Section 7 (Re-certification by Parent/Guardian Form) and where appropriate a Section 8 (Re-certification by Licensed Physician of Medicine or Osteopathic Medicince).
Section 7 Re-certification forms will be completed as part of the season registration process through FamilyID.
